New Hire Employee Packet

Upon employment the following documents are required for your New Hire Appointment with the HR Department.

Required Documents:

  • Drivers License
  • Social Security Card (For Payroll)
  • Transcripts or Diploma from college/university (teacher assistants)


Please fill out completely and bring to your appointment (Use Legal Name that matches Social Security Card, No Nicknames).
